Acute Biliary Pancreatitis

چکیده
More than 220,000 patients are admitted to hospital each year with acute pancreatitis in the United States alone.1 The most common etiology of acute pancreatitis is gallstones. This chapter will focus on diagnostic and management issues related to acute biliary pancreatitis (ABP). Differentiating gallstone-induced acute pancreatitis from other etiologies (alcohol, medication induced, hypertriglyceridemia) should be prioritized in the initial investigation of acute pancreatitis as ABP has its own specific management considerations.

Background and Aim: Acute pancreatitis is an inflammatory pancreatic disease that is associated with low or no fibrosis of the pancreas. Standard non-surgical treatment in all patients involves correcting water and electrolyte disorders, pain relief and avoiding pancreatic secretion. Given that pancreatitis is an inflammatory process, and NSAIDs have been shown to modulate inflammatory processe...
متن کاملClinical Diversity in Biliary Pancreatitis — Classification of Two Types
One hundred and seven patients with biliary pancreatitis undergoing operation from 1976 to 1989 were reviewed. To clarify the reason for failure to respond to conventional supportive therapy, 73 patients (68%) who underwent emergency surgery were retrospectively divided into two groups according to the severity of the pancreatitis evaluated at laparotomy and compared. Sixty-two had minimal or m...
متن کاملRole of biochemical investigation in prediction of biliary etiology in acute pancreatitis.
INTRODUCTION Acute pancreatitis is common clinical presentation. Gall stone disease and alcohol consumption are most common etiologies. Detection of biliary etiology is important in order to provide definite management in form of cholecystectomy to prevent further attacks. Sensitivity for abdominal ultrasound to detect cholelithiasis is decreased to 67-87% in presence of acute pancreatitis. Dif...

BACKGROUND Biliary tract involvement in acute necrotizing pancreatitis is rare. CASE REPORT We report a case of a 53-year-old man who had a pancreatic choledochal fistula complicating acute necrotizing pancreatitis. The fistula was suspected at computed tomography and confirmed at surgery. The patient underwent necrosectomy, cholecystectomy and proximal biliary diversion. He is well at 1-year...

UNLABELLED Background/ Aims: Despite the existence of an easy tool to diagnose biliary tract disease as an etiology for acute pancreatitis (AP), the sensitivity of abdominal ultrasound is around 80%, which can be even lower in certain conditions. METHODOLOGY We have retrospectively reviewed data of 146 patients admitted for acute pancreatitis between 1999 and 2013. Bivariate analysis for clin...
